Getting you appointed through Cavalier Insurance Marketing Services:
 

Step 1 of 2. Completing the Cavalier Associates Advisor Packet

Below is the Cavalier Associates advisor packet, this is a one-time packet we require all advisors to complete. Please return via email to cristina@cavalierassociates.com or fax to 805.367.8664

Step 2 of 2.
Completing your Carrier Appointment Request

Please click on the following link: https://surelc.surancebay.com/sbweb/agency/499
This will take you to our licensing portal.

For first-time users, click “REGISTER NEW ACCOUNT” when you get to the login page.
The following screen will ask you to enter your social security number and information.

For returning users, just enter your User ID (Email) and Password. Once you’ve logged in click on “My Info” and begin completing each section. After each step is fully completed and you’ve requested the carriers you’d like to be appointed with, we will be notified and able to finalize the appointment request.


NOTE: Please save your login information, as you will need this to request any future and/or additional carrier appointments.
